Adventure Program Acknowledgement
*
Collie Farm Stays is a real working farm, and our adventure program includes bushwalks, lake swims, play with other Collies, and time in nature. We take every care to keep each guest safe, but with this kind of active, outdoor experience, there is always a chance of minor scratches, bumps, mats or knots from swimming, or play-related scuffles. By choosing to participate, you acknowledge these natural risks and agree that Collie Farm Stays cannot be held liable for minor injuries, incidents, or grooming-related impacts that may occur during your Collie’s stay. By agreeing below, you voluntarily accept these risks and release Collie Farm Stays from any claims or liability arising from such events. If you feel uneasy or very protective of your Collie, this may not be the right program for you, and that’s completely okay.
I understand and accept the nature of the program and am happy for my Collie to participate
I understand but am not comfortable with this and do not wish to proceed.
Thank you so much
This form only needs to be completed once every twelve months to keep your Collie’s information up to date.
These details help us tailor each stay so your beautiful Collie feels safe, happy and completely at home while they are with us.
Just a gentle reminder, if we do not have a current C5 vaccination record on file, we will need that provided before your Collie’s next visit.
Did you know we also offer services in the City to keep Collies engaged and stimulated throughout the week. And if you are looking to bring a bit of the farm home, we have some beautiful products available too, including our much loved Border Bites, custom Photobooks and Collie Balls™.
If you ever need help or have any questions, we are always happy to chat.
